Joining Hands to Prevent Readmissions
It's no longer enough for hospitals to concentrate their readmission prevention efforts on what goes on inside the hospital walls. As the Centers for Medicare & Medicaid Services and other payers start to issue penalties for excess readmissions, hospital case managers need to collaborate with their counterparts at other levels of care to ensure that patients have a successful discharge.
