Information that can help your patients
Here are some examples of free educational tools home care nurses can use to teach diabetes patients about how to manage their disease:
1. "Daily Meal Planning Guide," published by Eli Lilly and Company of Indianapolis, contains a foldout guide to healthy eating for diabetics. It lists the various types of carbohydrates and recommended serving portions, and it teaches people how to count carbohydrates, exchange meal plans, and follow a food pyramid, which is illustrated on the tool. It has a small space for writing a personal meal plan.

2. "Diabetes Numbers At-a-Glance" is a laminated chart produced by the National Diabetes Education Program, which is a partnership among the National Institutes of Health, the Centers for Disease Control and Prevention, and more than 200 organizations.
The chart, directed to health care professionals, was printed through support from Lifescan, a Johnson & Johnson company, and includes information on diagnosing criteria for diabetes, glycemic control goals for type 1 or 2 diabetes, hypertension goals, the category of risk based on lipoprotein levels in adults, treatment decisions based on LDL cholesterol levels in adults, and management schedule.

3. "Fast Food Guide," a thick booklet by Becton Dickinson Consumer Health Care of Franklin Lakes, NJ, contains a breakdown of carbohydrate grams, calories, cholesterol, fat grams, sodium grams, and other information about a variety of fast foods from restaurants ranging from Arby’s to White Castle.

4. "Take Control of Your Diabetes: Start a Healthy Diet," a single-page education sheet created by Novo Nordisk Pharmaceuticals Inc., and Schering Diabetes, includes sample diets and examples of starchy foods, vegetables, meats, fats, milk, and free foods, which do not count toward a diabetic’s daily carbohydrate count.

5. "The Type 2 Diabetes Meal Planner" education sheet explains how good meal planning can help a diabetic gain better control of blood sugar levels. Produced by Bristol-Myers Squibb Co. of Princeton, NJ, this one-page tool also contains sample meal plans and examples of various types of foods and their recommended serving sizes.

"Nutritive and Non-nutritive Sweeteners," "Basic Meal Planning," and "Are You at Risk?"
