Maternal Infant Perinatal Pediatric Home Care Services
Pediatric Nurse is on call 24 hours/day, 365 days/year
Maternal/Infant Care
Home teaching and assessment designed for the new mother, especially high-risk OB patients and those leaving the hospital less than 24 hours after birth.
• Skilled nursing assessment of mother and baby.
• Home environmental assessment.
• Instruction in newborn care.
• Instruction in feeding (breast or bottle), dietary and fluid requirements, and restrictions.
• Instruction in perineal, episiotomy, and cesarean care.
• Instruction in follow-up medical care for mother and baby.
• Instruction in emergency care.
• PKU measurement, as necessary.
• Lactation consultation from certified lactation consultant, if requested.
• Home phototherapy for hyperbilirubinemia.
• Instruction in safety of infant and child abuse prevention.
• Referral to community resources.
Pediatric Care
Home teaching and assessment designed for the special needs child with acute or chronic illness, including nursing care and/or rehabilitation services. Care focuses on family support, confidence-building, and integration into community resources.
• Skilled nursing assessment of child related to medical plan of care and development.
• Instruction to parent, caregiver or child (as age-appropriate) in special care needs and disease process, medication management, and symptom control.
• Home safety assessment and safety management.
• Pulse oximetry, oxygen administration, and respiratory assessment and instruction.
• Instruction in special equipment such as C-R monitor, suction machine, oxygen, ventilator, and parental or enteral therapies.
• Instruction in emergency procedures, and when to contact the pediatrician.
• Referral to and coordination with physical, occupational and/or speech therapy professionals, as required.
• Regular reports to the pediatrician.
• Referral to community resources.
Pediatric Respite Care
Skilled nursing and/or home care aide services on an hourly basis, as required, to provide respite and support to parents or primary caregiver responsible for chronically ill, or technologically dependent child.
• Skilled nursing care, as required, by a comprehensive medical plan of care, including tube feedings, infusion therapy, ventilator care, oxygen administration, etc.
• Nursing assessment and intervention, parent teaching, and monitoring.
• Medication and special equipment management.
• Home health aide services, as appropriate, for personal care and play therapy.
• Regular reports to the pediatrician and others involved in the child’s medical care plan, including referrals, as appropriate.
• Coordination with schools and other community agencies participating in the plan of care.
Other Specialty Programs
• Antepartum care, i.e. hyperemesis, pregnancy induced hypertension, gestational diabetes.
• Preterm labor management in collaboration with infusion therapy provider.
• Dietary consultation from registered dietitian
• Medical social services to evaluate home situation and coordinate community care resources.
• Rehabilitation services (PT, OT, ST) for children over age 3 years. (Others are referred to community resources such as Kapiolani Mobile Therapy or DOE.)
