Drug Criteria and Outcomes-Treatment for H. pylori Infection
Preferred regimens: Three drugs for one week
Clarithromycin 500 mg twice daily + amoxicillin 1 g twice daily + omeprazole 20 mg twice daily (or lansoprazole 30 mg twice daily) for seven days
Clarithromycin 500 mg twice daily + metronidazole 500 mg twice daily + omeprazole 20 mg twice daily (or lansoprazole 30 mg twice daily) for seven days (for penicillin-allergic patients)
Alternative regimen: Four drugs for two weeks
Metronidazle 250 mg four times daily + bismuth 2 tabs four times daily + tetracycline 500 mg four times daily + famotidine 20 mg twice daily or cimetidine 800 mg twice daily or omeprazole 20 mg twice daily or lansoprazole 30 mg twice daily for 14 days
Alternative regimen: Two drugs for four weeks
Penicillin-allergic patients with contraindications to metronidazole therapy: Omeprazole 40 mg twice daily (or lansoprazole 60 mg once daily) + clarithromycin 500 mg three times daily for 14 days followed by omeprazole 20 mg once daily (or lansoprazole 30 mg once daily) for 14 days (total of 28 days)
