· OASIS Basics: Beginning to Use the Outcome and Assessment Information Set, produced by the Center for Health Services and Policy Research, 1355 South Colorado Blvd., Suite 306, Denver, CO 80222. The packet includes a book and a diskette for $68. It gives a detailed description of the specific components of the data set and has an example of a start-of-care record incorporating the OASIS items. Agencies can obtain a faxed order form by calling the center at (303) 756-8350.
· Outcome-Based Quality Improvement Manual, published by the National Association for Home Care, P.O. Box 91486, Washington, DC 20090. The book sells for $65 for members and $130 for nonmembers (shipping and handling costs $8). The Center for Health Services and Policy Research also will fax this order form. · Also, the Affiliated Community Visiting Nurse Association Inc. in Rockland, MA, is marketing its scannable OASIS assessment tools for nursing, physical therapy, and speech therapy at a price of $3,900.
