CCMC approves nutrition care in home study
The Commission for Case Management Certification in Rolling Meadows, IL, offers continuing education credits for nutrition home study for the following manuals produced by the Nutrition Screening Initiative, a project of the American Academy of Family Physicians, The American Dietetic Association, and the National Council on Aging:
· Managing Nutrition Care in Health Plans. This manual provides the medical framework and practical guidance to establish a nutrition screening and interventions program for geriatric members of managed care health plans. The manual presents general guidelines on how to set up a nutrition care program and how to integrate it into current programs and systems. Included are steps for program planning, implementation, evaluation, and marketing, and profiles of nine health plans' nutrition screening and interventions programs. (5 hours)
· Keeping Older Americans Healthy at Home. This manual gives home care professionals practical steps and tips for developing and implementing a nutrition screening and intervention program in the home care setting. General guidance on how to screen, intervene, and train medical staff to carry out nutrition care is provided, as well as steps to evaluate and market nutrition care programs. (5 hours)
· The Role of Nutrition in Chronic Disease Care. This manual places a new emphasis on cost-effective nutrition care strategies that can have a profound impact on disease progression and patient recovery. The manual presents treatment strategies that health plans can tailor to enhance the provision of chronic care. The topics covered include cancer, chronic obstructive pulmonary disease, congestive heart failure, coronary heart disease, dementia, diabetes mellitus, failure to thrive, hypertension, osteoporosis, and pneumonia. (10 hours)
