Avoiding the Readmission Nickel and Dime Trap

CMS requires a number of discharge planning policies and procedures, which must be followed for all patients, not just those on Medicare or Medicaid. By the end of 2016, more than 2,500 hospitals will have forfeited $428 million because of high readmission numbers.
